#5999 Some cert commands are missing the --ca option

As part of the lightweight sub-CA feature, most cert commands gained a --ca option, used to specify the CA to use for the operation. Some commands were ommited, namely cert-status, cert-revoke and cert-remove-hold. These should be fixed to allows specifying CA as well.


jcholast: Note that this should be fixed in time for 4.4, as it is an backward incompatible change.
