ci hook stopped working after a while, after upgrading to 5.1.3, how can we debug this?

plugin is loaded and everything was just fine until some time after the upgrade it no longer is willing to drop down. database table looks correct and we have cleared redis session store and as well browser is fresh in private browsing mode.
